Incident report forms
A standard form staff fill out to record accidents, injuries, illnesses, or other unusual events at a restaurant or hotel.
What it usually contains
- Date, time, and exact location of the incident
- Names and contact details of those involved and any witnesses
- Description of what happened and injuries or damage observed
- First aid given, emergency services called, and manager notified
- Witness statements, photos, and follow-up or corrective actions
What the assistant uses it for
Use it to answer what information must be captured after a guest slip, employee injury, foodborne illness complaint, or property damage, and who signs off and when. It also shows the reporting chain and timelines for notifying management or insurers.
How it is versioned
A document of this type has exactly one current version at a time. Upload a revision and it becomes the version that counts — the one before it is kept and dated, but is no longer what your assistant answers from. Nothing is ever deleted, so you can always show what this document said on a given date.
