Loss run request templates

A standard letter or form used to ask a current or prior insurer for a policyholder's claims history, or loss runs.

What it usually contains

  • Named insured, mailing address, and policy numbers
  • Policy periods or number of years of history requested
  • Lines of coverage covered by the request
  • Signed authorization from the insured releasing the records
  • Where and how to send the report, plus a response deadline

How it is versioned

A document of this type has exactly one current version at a time.
