Standard fill-in-the-blank court filings a lender or finance company uses to start a lawsuit, most often to collect an unpaid debt.

What it usually contains

  • Case caption with court name, parties, and case number blanks
  • Numbered allegations covering the loan or credit agreement, default, and amount owed
  • Jurisdiction and venue statements plus attached exhibits like the note or account statement
  • Prayer for relief listing principal, interest, late fees, costs, and attorney fees
  • Verification, signature, and service-of-process blocks
